Craft Cafe, 311 St. Louis St., West Plains, inspected March 28, pre-opening. Two violations: a thermometer to measure temperature was needed in all cold holding units and paper towel dispensers were needed at all hand washing sinks. Reinspection scheduled for April 27. Approved for opening.

10Box, 705 Kentucky Ave., West Plains, inspected April 22. Eight violations: observed meat for sale past sell-by date; cutting boards in produce area with deep pits/scarring that cannot be effectively cleaned or sanitized; several shelves or braces in walk-in coolers with rust, needing replacement; excessive ice accumulation in walk-in freezer; hot water took over 1 minute to reach 100 degrees at produce station, must be readily available; no paper towels at meat department hand sink; no employee hand washing signs at all employee sinks in the building; and flooring in produce area and walk-in cooler, fan covers in all walk-in coolers and flooring in large walk-in freezer in need of deep cleaning. A follow-up was scheduled for May 27 with reinspection of individual items planned for May 22.

Aldi Foods, 1210 Missouri Ave., West Plains, inspected Feb. 26, 2024. One violation: Flooring in walk-in cooler was in need of cleaning under juice shelving due to a spill. Reinspection was planned for March 27.

Boys & Girls Club of the Greater West Plains Area, 613 W. First St., West Plains, inspected May 3. Two violations: temperature measuring device not seen in all prep coolers and prep area hand sink dd not have paper towel dispenser. Both items would be corrected immediately.

Captain D’s, 1402 Preacher Roe Blvd., West Plains, inspected April 26, no reinspection needed. Two violations: ice accumulation was seen in walk-in freezer that was in need of repair to a door/seal, and prep coolers/freezers with food debris were in need of cleaning. The latter was corrected onsite, and the former would be corrected immediately.

Casey’s General Store 1531, 403 Broadway, West Plains, inspected April 25, no follow-up required. Three violations: pizza not properly labeled with date/time and chicken for sale past use-by time, thermometer not seen in pizza prep table cooler, and no hand washing sign at prep area hand sink. Signage was given to be posted and all other issues were corrected onsite.

Casey’s General Store 1715, 816 St. Louis St., West Plains, inspected Feb. 27, no follow-up required. Two violations: improper dish washing at prep area, three-sink system should be filled with water for proper wash, rinse and sanitize; and walk-in cooler flooring in need of cleaning. Both corrected onsite.

Casey’s General Store 2860, 1510 Preacher Roe Blvd., West Plains, inspected April 19. Four violations: excessive food debris at prep/sandwich cooler prep table in need of frequent cleaning, hot water at prep area hand sink took over a minute to reach 100 degrees, prep area in need of cleaning due to food debris buildup on equipment and floor and ceiling tiles damaged from roof leak in need of repair/replacement. Reinspection date set for May 19.

Casey’s General Store 3260, 1702 Gibson Ave., West Plains, inspected April 25, no follow-up visit required. Two violations: walk-in freezer door in need of repair and incorrect dishwashing procedure, second (rinse) step not done. The latter was corrected onsite and the former would be fixed.

Cattlemen’s Steakhouse, 1208 E. Main St., Willow Springs, inspected May 24, follow-up inspection scheduled for July 9. Seven violations: food prep employees with food and drinks without lids and straws in prep area, wash cloths not stored in sanitizer when not in use, food boxes stored on walk-in cooler and freezer floors, paper towels in employee restroom not properly dispensed, prep area ceiling tile damaged and in need of repair and chemicals must be labeled and kept away from all food and food prep areas. Paper towel matter corrected on site; all other issues given deadline of July 9 for correction.

Chen’s Garden, 1705 Gibson St., West Plains, inspected April 24, no reinspection required as violations are corrected or will be prior to next routine inspection. Five violations: hair restraints not worn by all food prep staff, employee drinks in prep area without lid and straws, food product in storage not covered to prevent possible contamination, chicken on floor in prep area and scoops stored incorrectly.

Colton’s Steakhouse, 1421 Preacher Roe Blvd. West Plains, April 7 inspection of temporary booth for eclipse festival. No violations.

Creative Cakes 2, 1430 W. Eighth St., West Plains, inspected April 23, no reinspection required as business will call when water heater is repaired. Three violations: hair restraints not worn by all food prep staff, upright freezers with excessive ice buildup in need of defrosting and no hot water as water heater was not working properly. The plumber had already been called to repair the water heater, and the defrosting would be fixed ASAP. Hair net issue corrected onsite.

Crossroads Convenience Store, 1808 Broadway, West Plains, inspected April 24, no reinspection needed. Four violations: pizza cutter stored improperly on towel between uses, ice machine cover missing, missing and damaged ceiling tiles in prep and storage areas in need of replacement and back soda fountain and ice machine area in need of cleaning. The first issue was corrected onsite, the second and last issues will be corrected by next routine inspection and the third item will be corrected ASAP.

Dairy Queen, 1655 S. U.S. 63, West plains, inspected April 25, no reinspection required. Three violations: hair restraints not worn by all food prep staff, prep freezer in need of cleaning due to food debris buildup and water at dessert area hand washing sink took over a minute to reach 100 degrees. First two items corrected onsite, last item would be repaired as soon as possible.

Diamond Head, 1214 Porter Wagoner Blvd., West Plains, inspected April 24, follow-up scheduled for May 24. Six violations: food product stored uncovered in prep and walk-in coolers; food product stored on floor of prep and storage areas; salad bar products stored at 50 degrees, over maximum of 41 degrees; cleaning needed of walls in prep and storage areas, soda fountain heads and ice machine flooring area; excessive air gap at back door, prep area screen door torn and not closing properly, both allowing for entrance of pests and flies; and paper towels not properly dispensed at prep and employee restroom hand sinks. Family Counseling Center Turning Leaf, 1015 Lanton Road, West Plains, inspected April 29, no reinspection needed. Two violations: tiles missing from pantry floor, making it difficult to clean, and cleaning products stored on top of refrigerator instead of away from food to prevent contamination. First issue would be corrected and second was corrected on site.

Glenwood School Cafeteria, 10286 Highway 17, West Plains, inspected April 4, no foul-up required. One violation: handled scoops laying in bulk sugar container, must store handle up to prevent contamination. Corrected onsite.

Grizzly Bite at Grizzly Lofts, 665 Missouri Ave., West Plains, inspected May 7, no follow-up required. Three violations: clean dishes stored on towel, must air dry on nonabsorbent surface; ice scoop stored unprotected on top of ice machine; and paper towel dispenser at prep area hand sink not working correctly. First two items corrected onsite, paper towel dispenser will be replaced.

Hardee’s, 905 Porter Wagoner Blvd., West Plains, inspected April 30, follow-up inspection scheduled for May 30. Seven violations: food boxes on floor of walk-in freezer in danger of contamination; hot food holding temp 128 degrees for gravy, below minimum of 135; broiler vent hood and air covers in prep area in need of cleaning; excessive air gap with back prep area door propped open, keep closed to prevent pests; no hand washing signage at all employee sinks; floors throughout prep area in need of deep cleaning due to grease and food debris accumulation, including under all equipment, tables, shelving, fryer area, water heater drain area, and behind ice machine and drive-thru soda fountain; and cobwebs on dry storage shelving in need of cleaning. Holiday Inn Express and Suites, 1301 Preacher Roe Blvd., West Plains, routine food safety inspection conducted March 20, no follow-up required. No violations.

Hometown Diner, 3816 N. U.S. 63, West Plains, inspected April 23, no follow-up needed. Three violations: hair restraints not worn by all employees, unshielded light bulbs in prep/dish areas and no paper towels at prep area hand sink. Light bulbs will be shielded or replaced with shatterproof bulbs and the other issues were corrected onsite.

Howell Valley School cafeteria, 6461 ZZ Highway, West Plains, inspected April 25, no follow-up required. Two violations: Food boxes stored on walk-in freezer floor and ice scoops stored uncovered/unprotected in concession area. The latter was corrected onsite. The school will look into adding chest freezers to help with excess product storage.

Ice Queens Tropical Sno mobile, 2016 Crawford St. West Plains, inspected May 17, no follow-up required. One violation: unlabeled spray bottle was found in prep area. Keep all spray bottles labeled for easy identification.

Jimmie’s Family Restaurant, 805 Porter Wagoner Blvd., inspected May 3, follow-up reinspection set for May 28. Six violations: ash tray in dry storage room used fro smoking, which should only be done outside to prevent contamination; hair restraints not worn by all food prep staff; walk-in cooler temp showed temp of 50 degrees, higher than the maximum allowable 45 degrees for current equipment or 41 for replacement equipment; soda syrup area and ice machine in need of cleaning; no paper towels at back prep hand sink and employee restroom hand sink; and dish area wall in need of repair to be smooth, easily cleanable and free of holes. All items to be completed by reinspection.

JJ’s Not your Normal, 804 St. Louis St., West Plains inspected May 9, follow-up reinspection Aug. 9. Three violations: warmer with corn and other product holding at 118 degrees, and pulled pork in warmer dish at 125 degrees, both less than the 135-degree minimum; clean items stored on cloth towel; and several flies observed throughout prep area, fly traps needed and doors to be kept closed. The item storage issue was corrected onsite; other issues will continue to be monitored.

Jo Coffee Company, 905 Preacher Roe Blvd., West Plains, inspected April 30, no follow-up required. Five violations: hair restraints not worn by all food prep staff, clean equipment and utensils stored on cloth towel instead of air dried, ice scoop laying in ice machine, no paper towels in the employee restroom hand sink and coffee prep counter in need of cleaning. The first three items were corrected onsite, paper towels will be replaced and the counter will be cleaned as needed.

Junction Hill School cafeteria, 8004 County Road 3010, West Plains, inspected April 29, no follow-up required. One violation: plumbing leak at three-compartment sink in need of repair. Scheduled for repair at end of school year.

KB’s Lemonade, mobile, West Plains, preopening inspection conducted March 21, approved for opening with no follow-up needed. No violations.

KC’s Sports Bistro, 409 Washington Ave., West Plains, inspected May 1, follow-up inspection set for July 1. Five violations: employees not washing hands properly and hand sink blocked by cart filled with dirty towels; hair restraints not worn by all food prep staff; salad bar products stored at 51 degrees and prep cooler at 50 degrees, both higher than the maximum 41 degrees allowed, instructed to add ice to salad bar and repair cooler; thermometers not seen in all coolers/freezers; and no paper towels at prep area hand sink. Two violations: prep cooler at 48 degrees, above maximum of 41, and leak under basement sink area in need of repair. The cooler would be monitored and repairs done as needed, and the sink was scheduled for repair.

Ramey’s Supermarket, 1211 Parkway Center, West Plains, inspected April 29, follow-up scheduled for May 29. Sapz Thai, 929 Preacher Roe Blvd., West Plains, inspected May 3, no follow-up required. Three violations: food product boxes stored on floor in back storage area, placing them at risk for contamination; thermometers not in all coolers for temperature monitoring; and cleaning needed of fryer/grill area flooring and food equipment, and utensils due to food debris buildup. The thermometer issue was corrected onsite and the other items would be corrected ASAP.

Six violations: exposed wood in prep/dish areas in need of painting or sealing for cleaning purposes, deep cleaning needed of all prep/dish and storage areas prior to opening, paneling not properly secured to wall, unshielded light bulbs in dish area, ceiling tiles in need of replacement in area and equipment not in use stored outside facility in smoker area. Facility inspected again May 9, routine inspection with no follow-up needed. One violation: floor repair needed in service area of store for cleaning purposes. Repairs would be started.

Snappy East, 1243 St. Louis St., West Plains, inspected May 6, no reinspection needed. Two violations: soda fountain syrup storage room and walk-in cooler in need of cleaning and damage to walk-in cooler wall in need of repair for cleaning purposes. Will correct both issues prior to next routine inspection.

Two violations: thermometer not seen in upstairs fridge for proper temperature monitoring and deep freezer in need of defrosting due to ice accumulation. The first issue was corrected onsite and the second would be corrected as time allowed.

Three violations: wash cloths not kept in sanitizer solution while not in use, thermometers not seen in all prep coolers and shelves with rust must be smooth and easily cleanable.
