Big Lots, 1362 Preacher Roe Blvd., West Plains, inspected Dec. 14. No violations.

Burger King, 1317 Preacher Roe Blvd. West Plains, inspected Dec. 14, with a reinspection date of Dec. 28. Seven violations: cut vegetables in walk-in cooler and prep line items were not property labeled with date, time and use-by date; the broiler floor freezer lid was broken and in need of replacement; the ice machine cover was missing, a repeat violation; the vent hood was dirty and in need of cleaning; a leak was observed in the filtration area of the water lines in the storage area; the drive-thru window was seen propped open and the back door not fully closed, and they must be closed to prevent entrance of pests/insects; and flooring, equipment and shelving throughout the business was seen with grease/food debris and in need of a deep cleaning. Preliminary reinspection was scheduled for Dec. 21 regarding the broiler floor freezer lid replacement. All other violations were given until Jan. 13 to be fixed.

Cafe Whimsy, 1382 Bill Virdon Blvd., West Plains, inspected Dec. 15. Three violations: An employee restroom door did not have a self-closing device, no paper towels were at the prep area hand sink, and cleaning and organizing was needed throughout the prep/storage/dish areas. A reinspection date has been scheduled for Jan. 14 for all three issues.

Colton’s Steakhouse, 1421 Preacher Roe Blvd., West Plains, inspected Dec. 15, with a follow-up inspection scheduled for March 14, 2024. Five violations: no thermometer was available in the meat-cutting room/walk-in cooler, the walk-in cooler registered a temperature of 49 degrees, above the maximum temp of 41 degrees; air vents throughout the building were in need of cleaning or painting to be smooth and easily cleanable; lighting in prep areas is in need of repair for cleaning purposes; and broken flooring tiles in the prep areas and walk-in freezer were in need of replacement. The temperature issue in the walk-in cooler was fixed onsite when it was discovered a breaker had been tripped and a repair company come to correct the issue. All other violations were given a deadline of Jan. 14 to correct.

Ozark Cafe, 662 Missouri Ave. West Plains, inspected Dec. 15. Four violations: food stored uncovered in prep area freezer; food boxes on floor in walk-in cooler; excessive ice accumulation seen in walk-in freezer; and excessive grease buildup was seen in the prep area on the flooring in the grill/fryer area and walls and equipment there, plus the mixer area, needed deep cleaning. The first two items were corrected onsite, and a work order was placed that day for repairs to the walk-in cooler for the ice accumulation. A deadline of Jan. 14 has been set for the cleaning issue.

Primo’s Grill, 1603 Gibson Ave., West Plains, inspected Dec. 15. Three violations: Frozen items were seen thawing at room temperature instead of in the refrigerator or under cool, running water; cold-holding lid was off, bringing the temperature of the unit to 47 degrees, above the maximum temp of 41; and no paper towels at prep area hand sink. All three issues corrected onsite.

Ruby Tuesday, 1008 Worley Drive, West Plains, inspected Dec. 18. Five violations: poultry products stored above fish, risking cross-contamination; cleaning cloths not stored in sanitizer bucket when not in use; no hot water at the prep area hand sink with temps recorded at 90 degrees below the minimum temp, repeat violation; cleaning needed of dish area floors and equipment, prep areas behind equipment and bar area soda nozzle; and repairs needed to broken and missing floor tiles in prep and dish area and walk-in cooler, and to walls in prep and soda fountain areas. The food storage issue was corrected onsite. A deadline of Jan. 17 was given to correct all other issues.

Snappy Mart No. 11, 3675 U.S. 63, West Plains, inspected Dec. 18. Five violations: Employees did not wash hands properly and prep area sink did not have hot water, soap or paper towels; food in freezers stored without covers, risking contamination; cabinets at soda fountain and coffee area are in need of replacement or repair to make them smooth and easily cleanable; broken floor tiles tin the back storage area need repair or replacement; and fryer area and vent hood need deep cleaning. Reinspection scheduled for Jan. 17.

Steak N Shake, 989 Worley Drive, West Plains, inspected Dec. 18. Four violations: hair nets not worn by all food preparation staff, no hot water at prep area hand sink, back door to prep area needs door sweep to prevent entrance of pests/insects and no employee bandwashing sign posted in men’s restroom. A reinspection date has been set for Jan. 17.
